§ 1504. Permit application.

An individual or business seeking a sidewalk café permit or table and chair permit shall submit to City Staff the following documents, as determined by City Staff:

Application form.

B.

Responsible party agreement.

C.

Property owner consent.

D.

Site plan (per Section 1505, below), with photographs (3 minimum).

E.

Current certificate of insurance and endorsement.

F.

Letter of authorization from the Health Department (for outdoor food service).

G.

Application fee, as provided in Title 49 Tulsa Revised Ordinances (see Fee Chart in Manual).

H.

Extension of premises drawing to ABLE (if serving alcohol).

I.

Pictures or brochures of proposed furniture.
