§ 106. Certificate application.
A.
An application for a certificate shall be on a form furnished by the Director. It shall be verified by the applicant and filed with the Director and shall include documentary proof of meeting, or the ability to meet upon entering service, the requirements in Section 105 of this chapter. The application expires if the applicant has not completed the process and received a certificate within one hundred twenty (120) days from the application date, unless additional time is justified by the Director.
B.
The Director shall make or cause to be made such investigation as he may deem necessary regarding the character, experience and qualifications of each person with a direct interest in the applicant's business; provided, however, that such investigation shall include forwarding such person's fingerprints and relevant personal data to the Oklahoma State Bureau of Investigation and the City Police Department for investigation and report.
C.
Each application shall be accompanied by a Seventy-Five Dollar ($75.00) processing fee and a fee of Twenty Dollars ($20.00) per person for each background investigation in excess of two (2). In addition, every application shall be accompanied by a Nineteen Dollar ($19.00) fingerprint fee, assessed to defray the cost of processing and investigation.
D.
Each application shall designate the type of service or services for which the applicant is applying, either taxicab, paratransit service or utility vehicle for hire. If the application is for paratransit service, it shall further designate the type of such service, such as wheelchair transport, physically limited transport, horse-drawn carriages, peda-cabs, rickshaws, or other service.
