Chapter 1. ANNUAL PERMITS |
§ 103. Application for issuance of annual permit.
Unless otherwise directed or approved by the Mayor, an application for the issuance of an annual permit shall be submitted in letter form to the Deputy Director of Customer Services, Public Works Department, and shall contain, as a minimum, the following information.
A.
Initial applications shall contain:
1.
A comprehensive statement detailing the nature and extent of the work which will be performed, identification of the field of work (i.e., mechanical, electrical, building or plumbing) for which an annual permit is requested, and a full and complete breakdown of the gross square footage within the building or buildings in which all work authorized by an annual permit would be performed;
2.
A statement which identifies any unique needs of the applicant which would be met by the issuance of an annual permit;
3.
Any additional statements which the applicant may wish to make to demonstrate its qualifications for the requested annual permit; and
4.
A personnel roster which contains the names of the applicant's full-time employees who will do all work authorized by the requested annual permit, which shall individually identify:
a.
Trade specialties, by field and skill level, and
b.
License and/or registration numbers of all contractor, journeyman or apprentice licenses issued by the City of Tulsa.
B.
Renewal applications shall contain:
1.
A statement that there is no change in the information originally submitted by the applicant or, if any changes have occurred or are contemplated, a full description of all changes and proposed changes; and
2.
A current personnel roster containing all information as required in Subsection A.4., above.
